STANDARD MODEL RELEASE FORM (Updated Legal Ver. 3.0)

THIS IS A LEGALLY BINDING DOCUMENT. PLEASE MAKE SURE YOU READ AND FULLY UNDERSTAND IT BEFORE SIGNING.

I (Please print) ___________________________________________ (model), for good and valuable consideration, the receipt of which is acknowledged (TFP/CD or dollar amount) _____________, give to Richard Rasner (photographer), his legal representatives, successors, and all persons or corporations acting with his permission, unrestricted permission to copyright and/or publish photographic portraits or pictures of me including images in which I may appear fully or partially nude, and the negatives, transparencies, prints, or digital information pertaining to them, or in which I may be included in whole or in part, or composite, or distorted in form, or reproductions thereof, in color or otherwise, made through any media in their studio or elsewhere for art, or any other lawful purpose, UNLESS the following line is initialed below:

The portraits and/or digital media are NOT to be re-printed without my permission with the exception of the photographer�s portfolio (online or otherwise,) which I understand may be shown to clients and other persons not for financial gain. In exchange for these exclusive rights I have paid the photographer the sum of $100.00 per hour plus processing and printing fees in the amount of $____________ (U.S. Dollar amount.)

I further release, discharge, and agree to defend the photographer, his legal representatives, agents, licensees, successors and assigns, and all parties acting under their permission, or with authority from them, or those for whom they are acting, from any claims for remuneration associated with any form of damage, foreseen or unforeseen, associated with the proper commercial or artistic use of these images unless it can be shown that said reproduction was maliciously caused, produced and published for the sole purpose of subjecting me to conspicuous ridicule, scandal, reproach, scorn and indignity in accordance with all applicable laws. I waive any claim that I may have for alleged violation of privacy, defamation or libel by the use of such photographs.

I acknowledge that the photography session was conducted in a completely proper and highly professional manner, and this release was willingly signed at its termination. I hereby represent that I am over the age of 18 years and have read the authorization and release prior to its execution. I have not been induced to sign the same other than by the recited considerations, by any representation or statement made by the photographer, his agents, employees, or anyone acting on his behalf.

The photographer, as primary producer, hereby certifies to all users that the visual depictions covered by this release satisfy the standards under paragraphs (a) (1) through (a) (13) of Section 75.5 of the regulations implementing 18 U.S.C. 2257.
